Futures Essentials partners with healthcare organizations, community agencies, and Social Care Networks to extend food and essential support through trusted referral pathways.
Futures Essentials is an active participant in the regional Social Care Network — a coordinated system that connects healthcare providers, social service organizations, and community resources to ensure individuals and families receive the support they need.
Through the Social Care Network, healthcare providers and case managers can make direct referrals to our food and essential needs programs. Closed-loop referral tracking ensures that referrals are received, acted on, and followed up.
If your organization is already part of the Social Care Network and would like to begin making referrals to Futures Essentials, contact us to get connected.

Your organization identifies a community member in need of food or essential support.

Submit a referral through the Social Care Network platform or contact us directly.

Futures Essentials receives and processes the referral within 1–2 business days.

The member is contacted, confirmed, and onboarded for services.

A referral status update is sent back to your organization to close the loop.
Hospitals, clinics, and health systems that identify patients experiencing food insecurity can refer members directly to our programs, including our medically tailored food and Food Is Medicine initiatives.
We participate in regional Social Care Networks, enabling closed-loop referrals and coordinated care for individuals and families connected to community health workers and case managers.
Nonprofits, faith communities, schools, and social service agencies can partner with us to extend food and essential support to the populations they serve.
Health plans interested in addressing food insecurity as a social determinant of health can work with us to design member food benefit programs and Food Is Medicine interventions.
If your organization is interested in partnering with Futures Essentials — whether to make referrals, co-design a program, or explore a formal partnership — fill out the inquiry form and a member of our team will follow up within 2–3 business days.
